The left-back was sent-off in the ninth minute of Saturday`s 4-1 thrashing at lowly Barnet - the Cobblers' fifth red card in the last nine games - and serves a one-match ban.
Veteran Marcus Hall will come back into the side as a ready-made replacement while manager Ian Sampson will have striker Leon McKenzie available.
McKenzie scored at Underhill but was replaced at the break as Sampson saved his legs for the trip to the Don Valley Stadium.
Sampson may be forced to make changes in midfield after playing for 80 minutes with 10 men so Kevin Thornton, Paul Rodgers and Michael Jacobs could all come in.
Striker Tadhg Purcell is out for the rest of the season with serious knee ligament damage.
